Assumption of Risk:

I, the undersigned, hereby acknowledge and understand that participating in martial arts training and related activities at Prime8 Martial Arts (hereinafter referred to as "the School") involves inherent risks, including but not limited to physical injury, psychological injury, pain, suffering, illness, disfigurement, temporary or permanent disability (including paralysis), and death.

Release and Waiver:

In consideration of being permitted to participate in any classes, training sessions, events, or activities at the School, I, on behalf of myself, my heirs, personal representatives, or assigns, hereby release, waive, discharge, and covenant not to sue Prime8 Martial Arts, its instructors, agents, employees, owners, and other participants from any and all liability, claims, demands, actions, and causes of action whatsoever arising out of or related to any loss, damage, or injury, including death, that may be sustained by me, whether caused by the negligence of the School or otherwise, while participating in such activities or while on the premises where such activities are conducted.

Consent to Receive Communications:

By agreeing to this waiver, you expressly consent to receive communications from Prime 8 Martial Arts, including but not limited to emails, SMS/text messages, and other electronic communications. These communications may include, but are not limited to, updates on classes, events, promotions, and important notices regarding your membership or participation in our programs.

You understand that you can opt out of receiving these communications at any time by following the unsubscribe instructions provided in the communication or by contacting us directly. However, opting out may limit your access to important information regarding your participation in our programs.

This section ensures that you have consent to send marketing or informational messages to your members while also giving them a way to opt-out if they choose.

Indemnification:

I agree to indemnify, defend, and hold harmless Prime8 Martial Arts, its instructors, agents, employees, owners, and other participants from and against any and all claims, liabilities, damages, losses, expenses, and costs (including reasonable attorneys’ fees) arising out of or in any way connected with my participation in the activities offered by the School.

Medical Treatment:

I understand that martial arts training is physically demanding and requires a level of physical fitness. I certify that I am in good health and have no medical condition that would prevent my participation in martial arts activities. I consent to receive medical treatment deemed necessary if I am injured or require medical attention during my participation in the activities. I understand and agree that I am solely responsible for all costs related to medical treatment and transportation.

Photo and Video Release:

I grant Prime8 Martial Arts the right to use, publish, and reproduce photographs, video footage, or other likenesses of me for promotional and educational purposes without compensation.

Governing Law:

This Waiver and Release of Liability shall be governed by and construed in accordance with the laws of [State/Country], without regard to its conflict of law principles. Any legal action or proceeding arising under this Waiver shall be brought exclusively in the federal or state courts located in [City, State/Country].

Sever Ability:

If any provision of this Waiver and Release of Liability is held to be unlawful, void, or for any reason unenforceable, then that provision shall be deemed severable from this Waiver and Release of Liability and shall not affect the validity and enforceability of any remaining provisions.

I have read this Waiver and Release of Liability and fully understand its terms. I acknowledge that I am signing this agreement freely and voluntarily and intend by my signature to be a complete and unconditional release of all liability to the greatest extent allowed by law.
